How do dermal fillers work?
Dermal fillers, also known as soft tissue fillers or facial filler, are a type of injectable treatment. Unlike Botox, an injectable procedure that works to smooth lines by reducing muscle movements, dermal fillers are used to add youthful volume beneath the skin. Surprisingly, there are many signs of aging that can be addressed by strategically adding volume with injectable fillers.
You can smooth deep lines and folds — including smile lines (or parentheses lines), marionette lines, and nasolabial folds — by getting filler injections beneath the creases. You can plump up thin lips with lip fillers and sagging cheeks with cheek injections to either restore fullness or create a new look. You can also use dermal fillers to add definition to a receding chin or soft jawline — and with some injectable fillers, you can even help keep your skin hydrated for longer periods of time.
There are a variety of injectable filler treatment options on the market, all of which offer different ways to enhance facial features. There are several different active ingredients used, with some even working to trigger new collagen and elastin production for lasting improvement to signs of aging. However, the most common facial fillers are made with hyaluronic acid.
How is hyaluronic acid helpful?
Hyaluronic acid is a type of sugar molecule and a naturally occurring substance in the human body (more specifically, it's found in your connective tissue). You've likely seen it in skin serums that are marketed as hydrating — that's because hyaluronic acid can draw in and hold up to 1,000 times its weight in water. It's this characteristic of the ingredient that makes hyaluronic acid-based fillers so exceptional at plumping up the skin.
Hyaluronic acid fillers come in different viscosities and weights to achieve different looks and goals. Regardless of your choice of filler treatment, though, it will refresh and revitalize your appearance via hyaluronic acid's ability to hold moisture and plump up sagging skin.
At Facet Dermatology, we offer Juvederm dermal fillers to deliver safe and efficient results using hyaluronic acid.
What are biostimulatory fillers?
Biostimulatory fillers are a unique type of dermal filler that trigger your fibroblasts, the skin cells responsible for collagen production. Over time, treatment with biostimulatory filler helps strengthen the skin, firming it and smoothing it for a more youthful appearance. With regular treatments, you may find that you need less and less filler to maintain your results.
The biostimulatory fillers we offer at Facet Dermatology are Sculptra and Radiesse. They can both be used in a variety of different ways to treat multiple concerns. Radiesse can be used to rejuvenate aging hands with injections in the backs of the hands as well as treat signs of aging in the chest, while Sculptra can be used to treat cheeks, arms, and even the buttocks for a nonsurgical butt lift.

What kind of improvements can be made with dermal fillers?
Because the types of facial fillers and the materials used to make them are so diverse, many different signs of aging can be addressed with injectable treatments.
Injectable fillers add volume under the skin. They're perhaps best known for transforming thin lips into a fuller pout. (Lip enhancement with filler can also improve the look of fine lines around the mouth, such as "lipstick lines" or vertical lip lines.) It can also plump up the look of sagging or flattened cheeks that may have deflated over the course of the aging process.
Because filler can be used to build out a soft jawline or small chin to create more definition, it's a great way to balance your facial features and create a more harmonious appearance without the need for plastic surgery. This kind of treatment is often paired with cheek filler injections to fully enhance your facial contours.
Filler injections administered under deep lines and severe wrinkles and facial folds around the nose and mouth, such as smile lines, marionette lines, and deep nasolabial folds. In some cases, severe frown lines (frown lines are the vertical lines between the eyebrows) can also be plumped up with dermal filler, but that area is typically best treated with a neuromodulator like Botox.
For those who have noticed signs of aging in their hands, especially thinning skin, some fillers can help rejuvenate that area for a more youthful appearance. Injectable filler is one of the only ways to truly restore a younger look to the backs of the hands.
For those who find they always look tired or sad, filler can also be injected in the hollows under the eyes, or tear troughs.
Lastly, there are dermal fillers that don't really add volume, but just use the powers of hyaluronic acid to restore deep hydration to aging skin cells. When your complexion is well-moisturized, your skin cells reflect light, creating a luminous glow and a youthful appearance.
What type of dermal filler do I need?
Dermal filler injections are not all the same; facial fillers have varying properties that make them particularly suited to certain uses or treatment in specific areas of the face and body. During your initial consultation, we’ll work with you to determine the best type of filler for you depending on your needs and desired outcomes.
Facial fillers such as Belotero Volume, Contour, and Shape and Juvederm dermal fillers like Voluma, Volbella, Volift, and Ultra Plus XC are well suited to address the loss of plumpness and sagging skin that naturally occurs as we age. These fillers can be used for lip injections, cheeks, temples, tear troughs (under eye hollows) and more. Belotero and Juvederm dermal fillers all use hyaluronic acid as the main ingredient, though they all have different functions.
In particular, Juvederm injection with Volux is specifically designed to sculpt the jawline and chin as it's considered a structural dermal filler. Juvederm injections with Voluma are best to enhance the facial contours of the cheek, Juvederm injections with Volift into the middle layer of the skin smooth out facial folds, and Juvederm fillers Volbella and Ultra Plus XC are best for lip enhancement (Volbella can also be used underneath the eyes).
Radiesse is a calcium-based biostimulatory dermal filler that is particularly well suited for hand rejuvenation; plumping the skin on the backs of the hands to reduce the appearance of visible veins.
Sculptra is a biostimulatory filler that can be used to help treat deep creases and folds on the face, smooth the neck, and create a more curvaceous butt through a nonsurgical butt lift.
Juvederm Volite is a unique injectable treatment. Instead of boosting volume, the hyaluronic acid in Volite boosts skin's moisture levels. By doing so, it improves the look of dryness, smoothing away crepey texture and fine lines, and delivers an overall glow. Volite can also be used for hand rejuvenation and neck lines.
Ultimately, you don't have to go into your appointment knowing which brand of filler or which type of filler you want or need. Dr. Yadav will assess your features and listen to your concerns, then determine which dermal fillers will help you achieve your desired look.
Are dermal fillers safe?
Dermal filler injections are approved by Health Canada and the FDA, and considered very safe when injected by highly trained and experienced practitioners. Potential side effects include mild redness, swelling, tenderness, itching and bruising at the injection site. The risk of allergic reaction in properly screened patients is low.
What to expect with dermal filler treatment
At your initial consultation for injectable fillers, you will discuss your concerns and goals with Dr. Yadav, who will then work with you on an individualized treatment plan. Depending on your schedule and desires, you may then be treated right after your consultation.
Though some fillers contain lidocaine, a pain reliever, Dr. Yadav may administer a local anesthetic like a topical numbing agent to reduce any other discomfort. Most patients find filler treatment to be largely comfortable, experiencing occasional mild discomfort at the initial injection. You may find that treatment sites around the lips are especially sensitive — that's normal! The lips feature many nerve endings, which can make treatment with lip fillers a bit more uncomfortable (conversely, many patients rarely feel cheek filler injections at all).
Filler is typically a quick treatment; depending on the number of treatment areas you've decided to address, you may be out of the office in 15 or 20 minutes. Dr. Yadav will give you an ice pack to alleviate any discomfort or swelling before you leave, and her office staff will set up a follow-up appointment with you so Dr. Yadav can determine if you need any further treatment or adjustments once your results have settled.
Is dermal filler right for you?
Dermal filler injections are an effective solution for both men and women looking to slow the signs of aging, smooth lines and severe facial wrinkles, restore lost volume, refresh their appearance, or enhance their natural features. You may not receive treatment if you are pregnant or breastfeeding.
How long will the results of dermal fillers last?
Everyone's experience with dermal fillers is different. How long the results of your treatment lasts depends on many factors, including your genetics, how quickly you metabolize filler, and the treatment area or areas that were injected. It's said that areas that move more frequently — such as the lips and the areas around the mouth — metabolize more quickly than areas that move less often (such as the chin, jawline, and cheekbones).
You can expect the results of your injectable filler treatment to last anywhere from six months to two years, depending on your unique experience. Because the results are not permanent (only plastic surgery can accomplish that!), the procedure can be repeated as desired so your facial features look the way you want them to look for as long as you'd like.
How long is the downtime after dermal filler?
There is little or no downtime recovery required after getting treatment facial fillers and you can return to most day-to-day activities immediately, though we recommend you avoid lying down and participating in any strenuous exercise right after your appointment to minimize discomfort. If you're going back to the office or to your home once you've left our office, however, you'll basically be able to carry on with life as usual.
The most common side effect of soft tissue fillers like Juvederm injection treatments is swelling. This is natural — it's your body's inflammatory response — and can last for a week or so. One of the best ways to reduce swelling is by sleeping elevated after your treatment. This will help any excess fluids drain and accelerate recovery. Drinking plenty of water can also help.
Another normal side effect is mild bruising at the treatment site, which can last for about a week after your injections. To minimize these effects, be cautious when taking blood-thinning medication ahead of treatment (this includes aspirin, ibuprofen, and supplements like fish oil). You should also avoid drinking alcohol in the 24 hours before your treatment as alcohol can also thin the blood. If you're on prescription blood-thinning medication, talk to your general physician, who can give you medical advice based on your specific health conditions.
Hold off on raising your blood pressure for at least 24 hours after your treatment with injectable fillers (we know, easier said than done). That's because increased blood pressure and flushing skin can increase your risk of bruising. This includes strenuous workouts and excessive heat (including participating in hot yoga or sitting in a steam room or sauna). Do your best to stay cool after your treatment.
Another thing to be cognizant of post-procedure is displacing your filler. While you shouldn't worry too much about this — sleeping on your face won't have any negative effects — you should not book any facials, massages, wear swim goggles (yes, really), or give yourself a gua sha treatment for at least two weeks after your dermal filler procedure. Filler needs time to settle where it's injected. Once you've passed that two-week mark, you'll be able to resume treatments or activities that may cause facial manipulation without worrying about your filler shifting.
Beyond careful prep for your procedure, you can help minimize mild bruising and swelling by applying an ice pack or cold compress to the injection area once you've been treated to help relieve and soothe any discomfort. Be sure to make sure you've effectively wrapped it a soft cloth. You want to avoid directly touching your skin with the cold item of choice. Skin is a very delicate organ; holding ice, frozen peas, and other objects directly to the skin could cause irritation and potential tissue damage.
Remember that you should always seek out a skilled and well-trained provider to perform your filler treatments. All aesthetic procedures come with risks and side effects, and those are heightened when administered by an inexperienced provider — and not all dermal fillers Toronto practitioners are qualified to administer injectables. Patients who book their facial filler treatments are in safe hands. Dr. Geeta Yadav is deeply experienced and highly trained and can deliver natural results with little risk.

Recovery & Results
Unlike treatment with neuromodulators, you don't have to wait around to see the results of your filler treatment: The payoff from dermal filler injections is instant. It is normal to feel “firmness” at the injection site for a few days after treatment. This area will soften and settle into place over time, leaving you with a soft and natural look. Results of your treatment may take up to 14 days to take full effect.

While they both use a fine needle and are temporary, prescription treatments to help improve the appearance of severe wrinkles and fine lines, soft tissue fillers (like Juvederm injections) and neuromodulators like Botox work differently.
Injectable fillers, such as Juvederm fillers, help restore lost volume by adding an injectable gel (typically made with the naturally occurring substance hyaluronic acid) under the skin. By plumping up the skin, facial folds get smoothed out. It can also enhance and define facial features — for example, create fuller lips or more chiseled cheekbones. Juvederm injections like Volite add extra hydration to the skin, which can help blur the appearance of fine lines and acne scarring. The results of facial fillers can last anywhere from six months to two years, depending on certain factors.
Botox and other neuromodulators help reduce movement in the muscles that create facial expressions, limiting their ability to contract and create lines in the skin. As the treatment sets in, your skin's texture becomes smoother, reducing the appearance of wrinkles and fine lines. Neuromodulator treatment tends to last about three or four months.
